How do I go about engaging the service?

Once you have taken a look at our website and made a decision you can download the form (please scroll down) which has all the information on it. Fill out the form as to the days and shifts you the service on. Forward this to our administrator ( vitalcare@tiscali.co.za) .

Once you have made a final decision then we require the following ;

Step 1. Complete the paperwork . Fill out the 2 pages of the on line booking form which will provide us with patient information and will indicate to us which shifts and days that you will choose to have care.

Step 2. Forward your Booking. E-mail your completed forms to vitalcare@tiscali.co.za. We cannot arrange for the paperwork to be collected but they may be couriered to 14 Marlborough Cres., Durban North for attention Charmaine Soobramoney.

Step 3. Medical Assessment. We will then book a compulsory medical assessment for the patient needing care for which an assessment fee does apply. (See below for assessment details).

Step 4. Paying your deposit. A fully refundable deposit will secure your service and needs to be paid in full before we commence. We cannot commence until this payment is made and cleared into our account. This deposit is refunded after your last invoice and final costs have been consolidated.
The Assessment

At the onset of care our registered nurse will make an appointment to spend some time with the client to do an in-home needs assessment. We always suggest that a family member or friend is involved too, so that we can understand their needs and expectations. It seems to be common nowadays for clients not to have local family available. We then try to involve family members from abroad, communicating via email, Whatsapp, face time or zoom.
It is helpful if our clients can provide us with chronic and/or acute prescriptions as well as any medical records or results they may have, as they may help us gain valuable insight into the client’s condition, treatment, and prognosis.
The clients mobility and accessibility in and out of the home, in the bathroom space, in and out of the bed and between rooms is a vital component of the assessment. Advice can then be given regarding appropriate mobility aids if need be. The Registered Nurse Assessor, can also advise on simple ways to modify areas to make care easier and safer, decrease the risk of falls and injury and keep the client living independently in their home for as long as possible.
